TO HELP OUR PATIENTS AVOID UNNECESSARY HOSPITALIZATIONS AND VISITS TO THE EMERGENCY ROOM, WE COORDINATE WITH PRIMARY CARE AND SPECIALTY PHYSICIANS, CONNECT PATIENTS TO COMMUNITY RESOURCES, AND PROVIDE EDUCATION ABOUT SERIOUS ILLNESS AND END OF LIFE PLANNING. IN ADDITION TO APRN HOME VISITS, WE OFFER A 24-HOUR CALL CENTER STAFFED BY RNS, RN AND SOCIAL WORKER TELEPHONIC AND HOME VISITS, AND A WELL-TRAINED OPERATIONS TEAM.
