THE MISSION OF BRAZOS VALLEY COMMUNITY ACTION AGENCY, INC. IS TO PROVIDE EVIDENCED-BASED HEALTHCARE UTILIZING A PATIENT-EMPOWERED TEAM APPROACH TO ACHIEVE INDIVIDUAL WELLNESS FOR ALL MEMBERS OF THE GREATER BRAZOS VALLEY COMMUNITY.

  • BRAZOS VALLEY COMMUNITY HEALTH CENTERS - THROUGH THIS PROGRAM, THE AGENCY DOING BUSINESS AS HEALTHPOINT, STRIVES TO ELIMINATE HEALTH DISPARITIES BY OFFERING QUALITY PRIMARY AND PREVENTIVE HEALTH CARE TO THE MEDICALLY UNDERSERVED IN ITS SERVICE AREA, INCLUDING HEALTH EDUCATION TO INDIVIDUALS. HEALTHPOINT SERVED A TOTAL OF OVER 45,000 UNDUPLICATED PATIENTS DURING THE YEAR. SERVICES INCLUDED PRIMARY AND PREVENTIVE MEDICAL, DENTAL, AND BEHAVIORAL HEALTH SERVICES. THISAMOUNTED TO OVER 150,000 ANNUAL PATIENT VISITS. HEALTHPOINT HAS FIFTEEN CLINIC LOCATIONS SPREAD OUT IN NINE COUNTIES IN EAST CENTRAL TEXAS, MOSTLY RURAL AND UNDERSERVED FROM A HEALTH STANDPOINT.
