Program accomplishments

  • CENTER-BASED THERAPY SERVICES - TEAMABILITY PROVIDES INTERDISCIPLINARY, CENTER-BASED OUTPATIENT THERAPY SERVICES TO CHILDREN, ADOLESCENTS, AND ADULTS WITH COMPLEX DISABILITIES. SERVICES INCLUDE PHYSICAL THERAPY, OCCUPATIONAL THERAPY, SPEECH-LANGUAGE THERAPY, CORTICAL VISUAL IMPAIRMENT (CVI) ASSESSMENTS, AND ASSISTIVE TECHNOLOGY AND AUGMENTATIVE AND ALTERNATIVE COMMUNICATION (AT/AAC) EVALUATIONS AND TRAINING. SERVICES ARE DELIVERED BY LICENSED CLINICIANS AND SPECIALIZED REHABILITATION STAFF IN A HIGHLY ADAPTED, STATE-OF-THE-ART CLINICAL ENVIRONMENT DESIGNED TO SUPPORT INDIVIDUALS WHOSE NEEDS CANNOT BE MET IN TRADITIONAL THERAPY SETTINGS. EACH INDIVIDUAL RECEIVES A PERSONALIZED PLAN OF CARE BASED ON COMPREHENSIVE, TRANSDISCIPLINARY EVALUATIONS THAT ASSESS FUNCTIONAL ABILITIES ACROSS MOTOR, SENSORY, COMMUNICATION, VISION, AND DAILY LIVING DOMAINS. CAREGIVERS ACTIVELY PARTICIPATE IN TREATMENT SESSIONS AND ARE TRAINED TO CARRY OVER THERAPEUTIC STRATEGIES INTO THE HOME AND COMMUNITY, STRENGTHENING LONG-TERM OUTCOMES AND FUNCTIONAL INDEPENDENCE. DURING THE YEAR, TEAMABILITY SERVED APPROXIMATELY 145 UNDUPLICATED PATIENTS, MANY OF WHOM RELY ON MEDICAID, MEDICARE, MILITARY-RELATED INSURANCE, OR COMPLEX STATE FUNDING SOURCES THAT REQUIRE SIGNIFICANT ADMINISTRATIVE COORDINATION BEYOND STANDARD CLINICAL CARE.
    Expenses $2,160,672 · revenue $1,108,224
  • CAREGIVER COACHING AND FAMILY SUPPORT - RATHER THAN GROUP WORKSHOPS, TEAMABILITY PROVIDES INDIVIDUALIZED, ONE ON ONE COACHING FOR PARENTS AND CAREGIVERS, EMBEDDED DIRECTLY INTO EACH CLIENT'S THERAPY PLAN. THERAPISTS WORK CLOSELY WITH FAMILIES DURING SESSIONS AND THROUGH PERSONALIZED COACHING CONVERSATIONS TO BUILD PRACTICAL SKILLS, CONFIDENCE, AND STRATEGIES THAT SUPPORT CARRYOVER AT HOME AND IN THE COMMUNITY. THIS HIGHLY INDIVIDUALIZED APPROACH ENSURES EDUCATION IS RELEVANT TO EACH FAMILY'S UNIQUE GOALS, CHALLENGES, AND ROUTINES, STRENGTHENING LONG TERM OUTCOMES AND EMPOWERING CAREGIVERS AS ESSENTIAL MEMBERS OF THE CARE TEAM.
  • SCHOOL & COMMUNITY COLLABORATION SERVICES - TEAMABILITY PARTNERS WITH LOCAL SCHOOL DISTRICTS AND COMMUNITY ORGANIZATIONS TO SUPPORT STUDENTS WITH COMPLEX DISABILITIES THROUGH SPECIALIZED EVALUATIONS, CONSULTATION, AND THERAPEUTIC COLLABORATION. SERVICES INCLUDE FUNCTIONAL ABILITY EVALUATIONS AND INTERDISCIPLINARY CONSULTATION DESIGNED TO INFORM INDIVIDUALIZED EDUCATION PROGRAMS (IEPS) AND EDUCATIONAL PLANNING. WHEN APPROPRIATE, STUDENTS PARTICIPATE IN THERAPEUTIC ACTIVITIES WITHIN TEAMABILITY'S SPECIALIZED CLINICAL ENVIRONMENTS THAT REPLICATE REAL-WORLD DEMANDS NOT EASILY ACCOMMODATED IN TRADITIONAL SCHOOL SETTINGS. THESE SERVICES SUPPORT GOAL ATTAINMENT, FUNCTIONAL PARTICIPATION, AND SKILL GENERALIZATION ACROSS ENVIRONMENTS WHILE COMPLEMENTING SCHOOL-BASED SERVICES.
  • PROFESSIONAL TRAINING, WORKFORCE DEVELOPMENT, AND COMMUNITY EDUCATION - TEAMABILITY PROVIDES PROFESSIONAL DEVELOPMENT AND WORKFORCE TRAINING FOR THERAPY PROFESSIONALS, EDUCATORS, AND STUDENTS SPECIALIZING IN COMPLEX DISABILITY CARE. THE ORGANIZATION SERVES AS AN OUTPATIENT CLINICAL TRAINING SITE FOR GRADUATE-LEVEL THERAPY PROGRAMS AND OFFERS CONTINUING EDUCATION OPPORTUNITIES FOR LICENSED PHYSICAL, OCCUPATIONAL, AND SPEECH-LANGUAGE THERAPISTS. TRAINING ACTIVITIES STRENGTHEN THE LOCAL WORKFORCE BY IMPROVING CLINICAL COMPETENCY IN SERVING INDIVIDUALS WITH SEVERE, MULTIPLE, AND LIFELONG DISABILITIES. TEAMABILITY ALSO COLLABORATES WITH LOCAL, STATE, AND NONPROFIT PARTNERS TO PROVIDE COMMUNITY EDUCATION AND CAREGIVER TRAINING AIMED AT IMPROVING ACCESS TO SERVICES AND LONG-TERM OUTCOMES.
