The community includes patients, caregivers, doctors, nurses, facilities, medical students and supporting staff of the Hospital. 0 0 339874 0 0 Family Center:The Patient and Family Support Center provides comprehensive, free support to patients' families during their children's hospitalization at University Pediatric Hospital. The center addresses the critical needs of families facing severe economic challenges-more than 50% of patients served come from families struggling to meet basic needs during hospitalization. According to Puerto Rico's Office of Management and Budget, 55% of children in Puerto Rico live in poverty. The center eliminates barriers that prevent families from thriving during their children's medical treatment, offering a welcoming refuge and constant source of support during the most difficult times. Program Activities includes distribution of essential supplies including hygiene products, clothing, diapers, and medical supplies, food vouchers for caregivers during extended hospital stays, entertainment resources: iPads, tablets, Nintendo Switch consoles, books, and games, three equipped playrooms with toys, electronic games, and activities, birthday celebrations and special holiday events for patients, communication support services, coordination with external organizations for additional services and resources and in-kind donation collection and distribution center. Population Served: Pediatric patients and their families/caregivers at Hospital Pediátrico Universitario, with priority given to long-term patients and families from rural areas outside the metro region. 291773 0 0 Child Life Program: The Child Life program provides specialized emotional and psychological support services to hospitalized pediatric patients and their families. It is the only certified Child Life program in Puerto Rico and the Caribbean. Services address children's emotional, developmental, and psychological needs during hospitalization through evidence-based therapeutic interventions, including medical play, preparation for medical procedures, and emotion management strategies. Program Activities include: individual assessments of emotional and developmental needs; therapeutic play and psychological preparation for medical procedures; creative therapies (art, music, dance); group and individual sessions in playrooms and patient rooms; crisis intervention and emotion management; psychological support workshops for caregivers; and collaboration with medical teams to integrate emotional support into care plans. Population Served: Pediatric patients hospitalized at Hospital Pediátrico Universitario and their families, across General Pediatrics (55%), PICU (19%), Surgery (14%), and Oncology (12%). Program Results (FY 2025-26): 1,272 patients treated, representing 31% of the 4,059 children admitted to HOPU. Interventions with patients included 2,085 screening visits, 353 therapeutic conversations, 318 creative-therapy sessions, 282 therapeutic games, and additional emotional support, music therapy, room enhancement, palliative care, preparation/education, recreational activity, and medical play sessions. Interventions with caregivers included 2,337 screening visits, 701 therapeutic conversations, 359 individual psychological sessions, and 61 therapeutic group sessions. 194107 0 0 Physical Therapy Program: The Physical Therapy program provides specialized rehabilitation services to pediatric patients from birth to age 21 who receive medical care in the hospital setting. The team of physical therapy specialists works collaboratively with the Department of Physical Medicine and Rehabilitation, focusing on pediatric-specific needs. The program promotes independence, optimizes functional mobility, and supports ongoing rehabilitation processes. Services address both physical recovery and emotional well-being, contributing to patients' sense of accomplishment, independence, and self-confidence. The program facilitates transitions back home and to school, provides referrals to other rehabilitation services when needed, and educates parents and family members with tools, resources, and guidance to support continued progress after discharge. Program Activities include: neurostimulation for neurologically depressed patients and gross motor development; therapeutic exercises and manual therapy for strengthening and mobility; treatment for orthopedic and post-surgical patients; activities of daily living including transfers and ambulation training; wheelchair and adaptive equipment training; caregiver education and home exercise programs; interdisciplinary collaboration with medical teams for comprehensive care plans; and training rotations for pediatric residents in neurodevelopmental approaches. Population Served: Pediatric patients hospitalized at Hospital Pediátrico Universitario across General Pediatrics (33.5%), Oncology (22.9%), PICU (19.5%), NICU (12.8%), and Surgery (11.4%). The financial committee is directed by the Board of Director's Treasurer. Form 990, Part VI, Section B, Line 12c The Board of Director and Committees held frequent meetings. The Board meets 8 times per year and the committees 18 times per year, to monitor execution of plans and compliance with policies and bylaws. External auditors also perform audit procedures to verify compliance and financial aspects. Form 990, Part VI, Section B, Line 15 The role of the finance committee is primarily to provide financial analysis, advise and oversight of Foundation's budget. Their sole responsibility is to ensure the organization is operating with the financial resources it needs to provide program and services to the community. Typical task areas for small and midsized groups include budgeting and financial planning, financial reporting and the creation and monitoring of internal controls and accountability policies. The committee determines the compensation of key employees based on industry standards and the operational budget. Officers are not entitled to compensation. Key employees such as the Executive Director have annual evaluations including compensation analysis. The Executive Director is evaluated by the President of the Board of Director. Other employee's evaluations are made by the Executive Director. Form 990, Part VI, Section C, Line 19 n/a The Fundación Hospital Pediátrico (the "Foundation") exercised ordinary business care and prudence in attempting to comply with its federal filing obligations. The timely filing of the Foundation's Form 990 was delayed due to circumstances beyond the Foundation's reasonable control, specifically the delay in completing the independent financial audit and issuance of the audited financial statements, necessary to prepare a complete and accurate Form 990. Section 6652(c)(1)(A) of the IRC imposes a penalty on any tax-exempt organization for failure to file a tax return or failure to include complete or correct information on the tax return. Section 6652(c)(5) states that no penalty is imposed if the taxpayer shows that the failure to file the tax return was due to a reasonable cause and not willful neglect. Reasonable cause exists when a taxpayer exercises ordinary business care and prudence but is nevertheless unable to file the return within the prescribed time. It has been interpreted that the term "willful neglect" means a conscious, intentional failure or reckless indifference. The Foundation did not intentionally disregard its filing obligation and did not act with willful neglect or reckless indifference. Rather, it continued working to obtain the financial information necessary to complete its annual return. Once the financial statements necessary to complete the return became available on September 15, 2026, the Foundation acted without further delay and submitted the return promptly. In light of these facts and circumstances, The Foundation respectfully requests that the IRS determine that the failure to file timely was due to reasonable cause and not willful neglect and grant full abatement of the penalty under IRC § 6652(c)(5).
