Form 990, Part VI, Section B, Line 11b The board of directors or an appointed comittee or individual selected by the board, reviews the financial statments and the IRS filings ontheir behalf. Most typically the board will select one member to meet with the Business Manager and the Executive Director to review the 990 filing. Form 990, Part VI, Section B, Line 12c All clinical and support staff as well as the board members are made aware of the policy and have access upon request to review it. All are encouraged review the policy and share any information they may have freely at regularly scheduled meetings and gatherings. All are expected to maintain the highly ethical practices within their scope of licensure or professional filed of practice. All records are available to the public by request. Documents include, but are not limited to: all govering documents, privacy policy, whistle blower policy, financial records, employee expectations, etc. A complete copy ov operating guidelines, corporate by laws, and all related policy and procedure information including history and function are supplied to each board member and again, available upon request to the public. Form 990, Part XI, Line 9 Other changes in net assets or fund balances of $88 are due to discrepencies in the amount of paid time off withheld and the amount paid out to the employee. The employee was paid more than what was withheld and it caused a change in the cash assets. We have adopted a record keeping system on a separate spreadsheet to avoid any errors in the future. Unfortunately, we have not yet recovered from the financial devastation due to the damages caused by the Covid epidemic. We have been unable to acheive our goal of hiring a second support staff person to alleviate the excessive workload of having just one support person responsible for the daily activities of billing and receiving to maintain the cash flow and the recordkeeping. Our current staff person, while capable of the recordkeeping required for filing, has been unable to do both in a timely manner. We appologize for the delay and know that our critical planning for more fund raising and financial gain to hire a second staff person or outside services is of the highest priority.
