Inadequate reimbursement for services, including disallowed and non-payments for responses we are statutorily obligated to provide, antiquated fee-schedules for Medicare and Medicaid, and arbitrary payment for service by commercial and private insurers continue to underfund and destabilize EMS service delivery. We anticipate fiscal constraints will continue to affect the EMS workforce, and erode sustainability of capital and operating programs. We anticipate a sustained period of volatility in healthcare demand, including in EMS service delivery.In coordination with our EMS partners, PVA continues to pursue reforms essential to payment for service, incorporation of telehealth and treat-in-place options, and strategies to align patients with non-ambulance services when more appropriate for patient needs. Students are recruited into each program using each partner's existing pathways. Recruited candidates are vetted for NYS EMS eligibility (background checks, etc.), and candidates are assessed for Monroe County residency as required. All qualified candidates associated with AMR and Monroe Ambulance are hired directly by these organizations, and each organization is responsible to report on student progress per terms of the subawardee agreement with each respective organization. Qualified expenses are invoiced to Pittsford Ambulance, and addressed per the agreement between Pittsford Ambulance and Monroe County. All qualified candidates associated with the MCC program enter into a grantee agreement between Pittsford Ambulance and the student. Qualified educational expenses are covered using grant funds, as defined in the agreements between Pittsford Ambulance and MCC, as well as Pittsford Ambulance and the grantee (student). Education expenses are invoiced to Pittsford Ambulance, and addressed per the agreement between Pittsford Ambulance and Monroe County. Education stipend expenses are managed by Pittsford Ambulance, and addressed per the agreement between Pittsford Ambulance and Monroe County. Ongoing student participation is a requirement of all students, regardless of which program they participate in. Students are obligated under their agreements to report any change in participation or eligibility for associated wrap-around benefits. As an added fiduciary layer, Pittsford Ambulance regularly monitors participation through direct observation and periodic (spot) reporting provided by the partner subawardees, on request. Students who are no longer eligible to participate in these programs are notified by letter, and funds are terminated with immediacy. Reporting on all activity is provided to Monroe County as required by the agreement between Pittsford Ambulance and Monroe County. Monthly, quarterly, and annual compliance meetings are conducted by Monroe County, during which all program activity and funds management actions are reviewed for compliance with all associated agreements and statutory requirements.
