We provided case coordination for over 264 individuals consumers this year. There were 414 measurable goals set and 398 specific measurable goals accomplished, 20 individuals received Advocacy services, 60 individuals received Assistive Technology service, 11 individuals received Housing services, 218 individuals received Independent Living and Life Skills, 105 individuals received Peer Consoling services, and no individuals received Personal Care services. A total of 1 individuals transitioned from nursing home to personal residences. Diversion service prevented 6 individuals from being place in an institution. Number of individuals received vocational services 0 and transition age youth, ages 18 to 26, 1. Also Community Access Center has over 12,565 community contact through information referrals, outreach programs, and public education.
