Ahli and Angela's marital status is fully disclosed, conflict of interest disclosures are signed, and neither party voted on any actions in regards to Angela's employment with the organization. Form 990, Part VI, Section B, Line 11b The board has access to the 990 form. We have a newly formed finance committee who can review the 990, if needed. Form 990, Part VI, Section B, Line 12c Our Board of Directors has an annual retreat and all new Board members are onboarded with our documents and disclosures. We revisit our Board documents at each retreat. In the midst of her leaving, we did not realize that the 990 was not submitted, even though we had compiled all of the information. If we could receive leniency with the late charges/interest, it would be greatly appreciated.
