In today's world, schools are charging families directly for most anything a child could want to do during the school day. Fees exist for advanced placement classes, field trips, science classes, lab fees, full day kindergarten, playing sports or musical instruments and many more. Families dealing with the high costs of cancer treatments and loss of income due to short or long term disability, if they are able to keep their job, are often faced with the decision of paying for their children's education or their life saving cancer treatment. We take away the stress of this decision, K-12, for our families and also provide a therapist to each family so the children have a voice outside the home during this difficult time. We are now working with 75 families in Colorado, 15 in Arizona, and 3 in Illinois, and 1 in Wyoming and 5 in Utah. We are registered in each state. As we now have operations in 11 states, we are starting to receive more grant funding in each of our states (AZ CO CT FL IL IN NV RI TX UT WY). When a grant is received in a state, it is added to our grants spreadsheet and then each disbursement is recorded against the grant until the funds are used in their entirety. Form 990, Part III, Line 2 in 2017 and 2018, we added families to the foundation's umbrella from Illinois. We registered with the Secretary of State in 2019, but have had issues with the state's requirement that we also register with the Attorney General in order to be able to solicit funds within the state. They were closed for many months during the Covid pandemic which made this even more difficult. In the end we have decided to focus on our continuing help to the families we work with in the state and keeping our registration up to date and will not be looking to solicit within Illinois. Form 990, Part III, Line 4a At the current time, the foundation's only program area is E86. We are currently working with 92 families in Arizona, Colorado, Illinois, Indiana, Utah, and Wyoming, and anticipate adding more in the remaining states we are currently registered in during 2024 and beyond. We are there for our families where a parent has cancer and the children are in danger of missing out on activities at school and also provide a voice for them outside the home. This has alleviated a great stress for the parents and we look forward to continuing our growth in the years ahead. Form 990, Part V, Line 1a The foundation is staffed entirely by volunteers, including the executive director, who believe all incoming monies should be focused on the families in need of our services as the mission is that important. He survives entirely on consulting fees earned throughout the year. Form 990, Part VI, Section A, Line 1a The board is currently comprised of volunteers with various skill sets to help us grow in the years ahead. Our growth into Arizona in 2017, Illinois in 2019, Wyoming in 2021, and Connecticut, Florida, Indiana, Nevada, Rhode Island, Texas, and Utah in 2022 has led to many prospects, but they remain independent of the board at this time. Form 990, Part VI, Section A, Line 8a The board meets quarterly to discuss the foundation's business and all meetings are open to the public. Notice is made on our social media as well as the website so that interested parties have the opportunity to attend. Minutes are taken for use by the foundation and its committees authorized to do business on behalf of the foundation. Form 990, Part VI, Section B, Line 10a The foundation opened branches in Arizona in 2017, Illinois in 2019, Wyoming in 2021, and Connecticut, Florida, Indiana, Nevada, Rhode Island, Texas, and Utah in 2022. Their operations mirror the way we do everything in Colorado as will other states that come on board in the years ahead. Form 990, Part VI, Section B, Line 11b The 990 is prepared by the Executive Director as he has intimate knowledge of operations in each of our locations. Once it is completed, it is sent to an independent accountant for review for reasonableness. Once approved it is sent to each member of the board for their input. Changes made by board input are then entered by the Executive Director and sent to the independent accountant for review and approval. Once approved they are then sent to the board prior to submission. Form 990, Part VI, Section B, Line 12c The foundation and the board affirm on an annual basis that they have operated within the guidelines of our Conflict of Interest Policy. In addition at each board meeting there is open discussion of the policy, and each member confirms their operations within its guidelines. Form 990, Part VI, Section C, Line 19 Our governing documents as well as our Conflict of Interest Policy and Financial Statements are available throughout the year to the public via the Colorado Secretary of State website and through direct request through the foundation's own website. Form 990, Part VII, Section A, Line 1a The foundation is staffed entirely by volunteers as we are mission driven and like to show that to potential funders as we grow. We are currently operating in 11 states with complete volunteer staffs and will keep this model in place as we continue our expansion. Form 990, Part VIII, Line 1c Once again our fundraising efforts were greatly impacted by Covid as we have stayed away from live events due to the very real danger that exists of bringing immune compromised people together with anyone else. As such, we have focused our energy on our existing donors and working on grants to make up for the missing live events and will continue in this manner for the foreseeable future. Form 990, Part IX, Line 25 Covid also impacted our expenses once again this year since we haven't done a live event since early on in 2020, it has changed the manner in which we raise funds which then impacts our expenses. Our expenses are certainly focused on our families and providing the assistance they need, but there are some more managerial expenses coming along that help us to continue our growth now and into the future. Form 990, Part X, Line 1 The foundation's assets consist mostly of cash and a few assets used in day to day operations. These assets are a printer, cell phone, laptops, tablets and other computer devices. Form 990, Part XI The foundation is keeping it simple in its early years of operation into our first 11 states as the need exists on a national level and we are focused on being that national foundation for all children of cancer patients. Form 990, Part XII The finance committee has overall oversight for the audit, which is a review at this time as well as review and compilation of the financial statements and selection of the independent accountant.
